This report analyses the tissue samples from the tumour(s) that were taken at your biopsies or surgical procedures.

• If you have had more than one biopsy or surgery, please be sure to get the reports for each of these procedures.
• The doctor who performed the biopsy or surgery is most likely to have this report.
• If you are unable to find a report - don't worry - we have a specialised team that is trained to recover reports and investigations rapidly.

These are tests that have been taken to produce images of the tumour(s). The images may be on disc or on film. Examples include x-rays, CT scans, MRIs, PET scans and bone scans.

• Call the hospital where the images were taken.
• Ask if the images are available on disc. If not, then ask for the actual films.
• Also ask for copies of the written reports for each test.
• If you need help, call the doctor who ordered the tests. He or she may be able to get these images for you.
• Again – don’t worry if you cannot get hold of all your x-rays. Our team will assist you wherever possible.

This is a description written by your doctor detailing your condition and general medical history, as well as the treatment you have received.

Please get medical summaries from your doctor(s) if:

• You have started cancer treatment before coming to DMO. Call the doctor who treated you and ask for a written summary or a copy of your records.
• You have ever seen a cardiologist or other doctor for a heart problem. In addition to the summary, ask for results from all recent tests you have had, including, for example, stress tests and echocardiograms.

Please be sure to bring a list of all medications that you take, including those prescribed by doctors and those that you buy over the counter (vitamins or herbal products). If you are unable to do so, please bring the labelled bottles or boxes with you to your consultation.

The list should indicate the following information:

• The name of each medicine.
• The dose you take
• How often you take it

This information is critical in ensuring that you get the correct treatment at all times. Please keep it with you for all consultation throughout your treatment and at all follow up consultations

We will need to know your medical aid name as well as who is the main member, which option you are on, and the membership number. We will also require proof of membership, so please be sure to bring your membership card or a letter from the scheme. This helps with authorisation checks and submission of your treatment plan at appropriate stages.

• Bringing written questions that you’ve thought of before the visit.
• Bringing paper and a pen so you can take notes during your appointment.
• Arranging for a friend or family member to come with you. You will receive a lot of information during the appointment and it’ll be helpful to have someone else with you to listen and take notes.
